岗位职责:

Primary Duties-
Outpatient:
1) Review and interpret Out Patient and Emergency Department electronic prescriptions and input into pharmacy computer if required
2) Evaluate and resolves real or potential prescription related issues.
3) Perform daily out patient dispensing function in accordance to Out Patient Pharmacy Workflow & Tasks i.e. return-to-stock & handle owe-out slips
4) Provide patient consultation on prescription, vaccines, and OTC medications.
5) Participate in pharmacy daily tasks (i.e. narcotic count, record keeping)
8) Able to provide drug information service to patients and medical staff.
9) Manage & control medication kept as floor stock at Out Patient Clinics & Emergency Department.
11) Responsible for at least 1 Quality indicator of the department.
12) Work as a team in the Pharmacy Department.

Inpatient:
1. Create new patient chart of medication order and conduct medication reconciliation for new admission.
2. Identify and document patient’s own medication if brought in to hospital
3. Review patient’s daily progress notes and lab result before participating nursing daily huddle/ ward rounding
4. Review all new physician’s orders including day-surgery cases.
5. Evaluate and resolves real or potential prescription related issues.
6. Dispense medication hospitalized patients to Patient Medication Cart.
7. Bedside dispensing for discharge patients and conclude inpatient medication bill on discharge.
8. Participate in pharmacy daily tasks (i.e. narcotic record keeping; weekly replenishment of infusion floor stock)
9. Actively participate in pharmacy activities and comply to good pharmacy practices.
10. Able to provide drug information service to patients and medical staff.
11. Manage & control medication kept as floor stock at inpatient wards & Operating Rooms.
12. Perform monthly In Patient Antibiotic Usage Analysis as part of Pharmacy Quality
Indicators.
11. Prepare guideline for admixture & administration of specific drugs.
12. Present at least 1 case study per month to department.
13. Support outpatient pharmacy on as needed basis.
14. Work as a team in the Pharmacy Department.

Other Duties Performed-
1) Take part in evening and weekend shifts.
2) Assist stock specialist in drug procurement (order, receipt and shelving) when required
3) Dispense and maintain proper stock of vaccines.
4) Compounding products for department stock assisted by Pharmacy Technician.
5) Responsible for special project as assigned by pharmacy manager.
